First fertility appointment: what to expect and what tests you’ll undergo

Taking the first step toward a fertility consultation can trigger nerves, doubts, and even fear. It’s completely normal to feel this way. Many women and couples arrive at their first assessment with questions about their reproductive health, how their body is functioning, or their real chances of achieving a pregnancy. That’s why understanding what this visit involves, what is evaluated, and what tests are included can give you peace of mind and help you feel prepared.
At Fertilidad Integral, our goal is to guide you from the very first minute with clarity, empathy, and a fully integrative approach. Your first appointment is a safe space to ask questions, understand your current fertility, and decide your next steps with confidence.
What is a fertility assessment appointment?
The assessment appointment is the initial consultation in which a Reproductive Biology specialist evaluates your reproductive health comprehensively. It’s the starting point for identifying possible causes of infertility, preventing complications, and determining the most appropriate pathway for you.
You are not expected to arrive with answers, this is the space where you’ll get them. The specialist listens to your story, reviews your background, and performs key tests that allow for an evaluation of your ovarian reserve, uterine health, and, if you have a male partner, semen quality.
What to expect during your first fertility consultation
1. Initial conversation and medical history review
The specialist will begin with an introductory conversation to learn about:
- Your reproductive age
- Menstrual cycles
- Previous gynecological symptoms
- Pregnancy history
- Past contraceptive methods
- Medical conditions or previous hormonal treatments
- Lifestyle habits
- Time trying to conceive
This conversation is not a test. It’s a space to express questions, fears, and expectations. Many patients arrive nervous, and that’s completely normal. At Fertilidad Integral, our team supports you with compassion, clarity, and genuine care.
2. Tests included in the first appointment
Your first consultation at Fertilidad Integral includes the fundamental tests needed to evaluate your current fertility. These allow you to receive answers from the very beginning and start with a personalized approach.
Evaluation ultrasound
A transvaginal ultrasound allows the specialist to review:
- Number of antral follicles
- Ovarian appearance
- Presence of cysts, fibroids, or polyps
- Endometrial thickness
- Uterine morphology
- Overall reproductive system health
This study is essential for performing the antral follicle count, one of the clearest indicators of ovarian reserve, and for evaluating uterine morphology.
Anti-Müllerian Hormone (AMH) test
This blood test measures the number of growing follicles inside the ovaries. It is considered one of the most reliable indicators of ovarian reserve, as it provides a close estimation of how many eggs could be available for a future pregnancy or assisted reproduction treatment.
The AMH test helps determine:
- The approximate number of eggs you currently have
- How you may respond to ovarian stimulation if you need a treatment such as IUI or IVF
- What type of treatment may be most suitable
- Whether there is a risk of low ovarian reserve or the need to act sooner
The test is simple, does not depend on the day of your cycle, and provides essential information to understand your fertility and plan your next steps with confidence.
If you have a partner: recommended semen analysis
If you have a male partner, we recommend a semen analysis (sperm test). This study is essential because it evaluates semen quality in detail and helps identify factors that may be affecting fertility.
A semen analysis evaluates:
- Concentration: number of sperm per milliliter.
- Motility: the percentage of sperm that move and how efficiently they do.
- Morphology: shape and structure of the sperm, which influence their ability to fertilize the egg.
- Vitality: percentage of live, functional sperm.
- Physical characteristics: viscosity, pH, liquefaction time, and volume.
This test provides essential information about the male factor. It’s important to remember that male factor is involved in 40–50% of infertility cases, so evaluating it from the beginning provides a more complete diagnosis and helps guide treatment appropriately.
What happens during your first fertility consultation?
Once your ultrasound, AMH, and semen analysis (if applicable) results are ready, the specialist compiles everything to offer a clear overview of your reproductive health. At this stage, you’ll be able to:
- Understand your current fertility level
- Identify possible factors that may be affecting conception
- Learn about your chances of achieving pregnancy naturally or with medical support
- Review treatment options and define a personalized plan
- Resolve any questions about your diagnosis or next steps
In many cases, these initial tests provide enough information to move forward. However, depending on each case, the specialist may suggest additional tests. Common complementary studies include: ureaplasma, mycoplasma, and chlamydia cultures; blood chemistry; TSH; and prolactin levels.

Frequently Asked Questions about the first fertility appointment
1. Do I need to come on a specific day of my menstrual cycle?
Not necessarily. Both the ultrasound and the AMH test can be done on any day of your cycle. If the specialist needs a test on a specific day (such as progesterone or estradiol), they will let you know after your check-up.
2. Does the first appointment include tests?
Yes! At Fertilidad Integral, the fertility check-up includes a transvaginal ultrasound and AMH, two key tests for evaluating your fertility. If you have a male partner, semen analysis is recommended.
3. What if I’m nervous or don’t know what to ask?
Totally normal. The specialist will guide you every step of the way. The appointment is designed to resolve doubts, explain concepts, and give you clarity without needing to arrive “prepared.”
4. How long does the consultation last?
On average, 1 hour, depending on your history and tests performed. The goal is that you leave with answers, not more questions.
5. Do I need to fast?
No. None of the tests included in the first check-up require fasting. If later you need specific bloodwork, the specialist will let you know.
6. What happens after the check-up?
The specialist will review your results, explain your fertility level, identify possible causes, and define the best treatment options. You’ll receive follow-up for your personalized plan and next steps.
